Tell us what you think about using online healthcare services
We’re working with the Health Inequalities Team at NHS Lincolnshire Integrated Care Board to understand how we can better support patients who use digital healthcare services.
We know that not everyone finds it easy to use things like online forms, booking systems or the NHS App. That’s why we’re asking for your feedback.
We’ve created a short survey to help us understand any challenges patients may face and find ways to make online healthcare easier and more inclusive for everyone.
The survey takes around five minutes to complete and is mostly multiple choice. You can fill it in online here.
Paper copies are available at reception and in the waiting room for anyone who cannot complete the survey online. Alternatively, if you are supporting someone who would struggle with the online version, we’d really appreciate your help in filling it out with them, especially if they are unable to get to the practice.
Your views will help us shape services that work better for all patients.
Thank you for your support.
